The patient was taken aback by a question which suggested, that instead of confirming her “illness,” the elopement may have manifested her way, albeit somewhat self-defeating, of problem solving. This suggested that instead of her “craziness” causing the elopement, it was really an indirect way of problem solving; and that is something healthy.

She looked up, thought awhile and finally said: “Maybe it was a sign that I’d better change my ways of dealing with the frustrations of institutional life...or else!”

The latter explanation was much more constructive than the former. A resident hearing only the former would just have reason to feel more helpless and depressed. However, building on the strength of the latter, s/he might begin to see him/herself as thoughtful, intelligent and proactive.

Hearing the latter theme in various conversations would tend to build an image of strength through its extraction of helpful meaning.

 Notice how her psychologist skillfully led her, without her knowing it, to positively reframe her problem situation so that she unearthed a strength: Being able to extract an important lesson, rather than using the problem to reinforce her identity of being “ crazy.”

Notice that the psychologist shaped this interaction so that it was “suggestive” of the resident’s mastery, rather than her incompetence or pathology. Can you see how the psychologist shaped the interaction so it went, at least, a little way towards changing her self-concept positively?
