Rights and Responsibilities
As a Humana plan member you have certain rights
and responsibilities when being treated by
Humana-contracted physicians. The rights and responsibilities statement reminds
members and
physicians of their complementary roles in maintaining a productive
relationship.
Humana plan members have the right
to:
• Be provided with information about your Humana
health plan, its services and benefits, its
providers, and your rights and responsibilities as a
member
• Choose your primary care physician from our
network of affiliated physicians and to change
to another primary care physician in the Humana network (For HMO plan members
only)
• Privacy and confidentiality regarding your
medical care and records. Records pertaining to
your health care will not be released without your or your authorized
representative’s written
permission, except as required by law
• Discuss your medical record with your physician
and receive, upon request, a copy of
that record
• Be informed of your diagnosis, treatment
choices, including non-treatment, and prognosis in
terms you can reasonably expect to understand, and to participate in
decision-making about
your health care and treatment plan
• Have a candid discussion with your practitioner
about appropriate or medically necessary
treatment options for your conditions, regardless of cost or benefit
coverage
• Expect reasonable access to medically necessary
health care services, regardless of race, national
origin, religion, physical abilities, or source
payment
• File a formal complaint, as outlined in the
plan’s grievance procedure, and expect a response to
that complaint within a reasonable period of time
• Be treated with courtesy and respect, with
appreciation for your dignity and protection of your
right to privacy
• Make recommendations regarding the plan’s
“rights and responsibilities” policies
• Receive Humana’s “Notice of Privacy
Practices”
• Expect your personal information to be kept
secure and used appropriately for payment and
health plan operations
• Expect Humana to adhere to all privacy and
confidentiality policies and procedures
• Expect the following activities concerning your
personal information –
– Request an accounting of disclosures of personal
health information for reasons outside of
payment and health plan operations
– Receive an authorization form for any proposed
use of your personal health information
outside of routine payment and health plan operations
– Request an alternate form of communication of
personal health information if the release of
a portion or all of the information could endanger life or
health
– Right to complain regarding an alleged breach of
privacy
– Right to agree or object regarding Humana’s
intent to release your personal information
outside of payment or health plan operations
– Right to request an amendment or correction of
your personal information to a designated
record created by Humana
Member Rights and Responsibilities
– Right to request access to inspect and copy
information
– Right to request Humana to restrict the use and
disclosure of your personal information and
the right to terminate the restriction request
Humana plan members also have the responsibility
to:
• Give the Humana plan and your health care
provider complete and accurate information
needed to care for you
• Read and be aware of all material distributed by
the plan explaining policies and procedures
regarding services and benefits
• Obtain and carefully consider all information
you may need or desire to give informed
consent for a procedure or treatment
• Follow the treatment plan agreed on with your
health care provider and weigh the potential
consequences of any refusal to observe those instructions or
recommendations
• Be considerate and cooperative in dealing with
the plan providers and respect the rights of
fellow plan members
• Schedule appointments, arrive on time for
scheduled visits, and notify your health care
provider if you must cancel or be late for a scheduled
appointment
• Express opinions, concerns, or complaints in a
constructive manner
• Tell us in writing if you move or change your
address or phone number, even if these changes
are only temporary
• Pay all copayments or premiums by the date when
they are due
• Be honest and open with your physician and
report unexpected changes in your condition in
a timely fashion
• Follow health care facility rules and
regulations affecting patient care and conduct
• Carry your Humana identification card with you
at all times and use it while enrolled in the
Humana plan

Dr. Mike Shery who is the director of ACRS and is a licensed
clinical psychologist. He has practiced clinical psychology for
approximately 24 years and is affiliated with almost all health plans,
including: ValueOptions, Medicare, Cigna, Cigna Behavioral Health,
United Health Care, Aetna, First Health, Healthstar, Blue Cross Blue Shield
of Illinois, ComPsych, Magellan Health, HFN, Tricare, Humana, most union
local plans, most school district plans, Unicare, ChoiceCare, CAPP,
Multiplan, PPONext, Private Health Care Systems, Humana-Military, Mental
Health Network, Managed Health Network, United Behavioral Health and Beech
Street.
He is board certified as a specialist in professional counseling by
the International Academy of Behavioral Medicine, Counseling and
Psychotherapy. He a member of the American Counseling
Association.
The office is located in Cary, IL, near Crystal Lake and
Algonquin, in northern Kane County and in southern McHenry County. In select
cases, phone consultations are available for those who don’t live
