Are you Getting All you Should from your Current Mental Health Provider?

Do you already have a provider?  Maybe you need another.  Let's see how your current mental health provider is doing…

 

Mental Health Provider Assessment

Does your provider keep to a regular schedule? Would many of the full time clinical staff members be able to say what days and times s/he is on site and be available for staff consults?

Does the provider seem to consult with a residents family in at least one out of four cases?

Does your provider actively participate in at least 75 per cent of your Resident Review meetings, providing practical and useful suggestions, information etc?

Does your provider systematically do an inventory of his patients strengths, resources and progress?  Does s/he provide that information to relevant staff and teach them how to build on them?

Does your provider proactively approach residents with dementia to interact in a way which is meaningful to them, rather than ignore or show no interest?

Does your provider exude warmth and approachability…rather than distance and aloofness?

Does your provider frequently meet with resident families to clarify and re-structure their expectations to prevent or address any disappointment and frustration?

Does your provider request information from staff about residents with psychiatric diagnoses or prescriptions for psychotropic meds who are not receiving any psychotherapeutic treatment, to monitor for any possible problem behaviors that are not being addressed?

Does your provider invite consultations regarding treatment resistant residents and formulate a plan in consultation with staff to resolve the resistance?

Does your provider initiate consultations and meetings with other professional staff to acquire information and coordinate a particular resident’s care?

Does your provider study the chart and ask for information to identify and inventory any resident behaviors that may demonstrate improvement or progress toward treatment goals?

Does the provider actively attend and participate in at least 75% of the meetings of the Psychotropic Review Committee?

Does your provider promptly complete documentation, enter it into the chart and provide it in a form which is informative?

When a problem with a specific resident arises, does your provider conduct a “mini-in-service” with her caregivers to arrive at a useful plan of action?

When a problem arises with a specific resident, does your provider assess triggers and train staff to recognize and prevent them?

Does your provider invite consultations regarding inappropriate or acting-out resident behavior?  Does s/he design behavior management plans with staff and monitor and update them?

Does your provider “touch base” with your head social worker and supervising nurse during each visit to prevent or address “loose ends.”

Does your provider teach staff how to adjust its behavior to minimize behavior problems which occur as a result of a resident’s fixed and immutable physical disability?

Does your provider invite questions from staff and have a store of resources, handouts etc. to provide?

Has your provider posted a list of symptoms at each nurses’ station which should trigger a referral?

Has your provider taught the staff how to identify improvements in resident behavior and emotional status?  Has the provider taught staff how to use those improvements to strengthen the resident’s course of treatment?

Does your provider have meetings regularly scheduled with the facility DON and social worker?

 

 

 

    

Does your provider have a working knowledge of the residents’ medical problems, their medications and their possible effect on resident mood and behavior?

To get a clear picture, does your provider first discuss a referral with the person making it, before proceeding with treatment or testing?

Does the provider appear to intermittently peruse the entire chart to identify any discrepancies that may be important?

Besides reviewing the chart, does the provider request information from staff to help in designing and implementing a treatment plan?

Besides reviewing the chart, does the provider request information from family to help in designing and implementing a treatment plan?

Does your provider respond rapidly to a referral or request for information?

Does your provider, after prescribing a course of action, follow-up and show an interest by requesting progress updates?

Does your provider, at the beginning of treatment, orient and educate families to reassure them and insure that their expectations are realistic?

Does your provider serve as a clarifying bridge between families and caregivers to maximize mutual understanding?

Does your provider arrange for a private setting in which to conduct the therapy sessions?
